How do I Become a Patient?
All new patients must register online, over the phone or in person at Olde Towne Medical & Dental Center prior to your appointment to become a verified patient.

Your mobile registration will be texted and emailed
to you 3 days prior to your scheduled appointment.

Insured Patients
All new patients must register in person at Olde Towne Medical & Dental Center. Patients with a Medicare, Medicaid, or insurance card must complete their patient registrations (below), and present all insurance cards with a valid photos ID to the front desk. An appointment will be given at this time.

I do not have a Medicare, Medicaid, or insurance card, but I am interested in the sliding scale.
Uninsured Patients
All new patients must register in person at Olde Towne Medical & Dental Center. Before your first appointment, you will need to present a completed registration form, and proof of income. (please bring to your first appointment in the form of a pay stub or bank statement).
Proof of income is used to qualify you for the sliding scale. If you are eligible, you will receive a card in the mail which you should bring with you to your appointments at OTMDC.
During registration, a prepay is collected and applied towards your copay. Your prepay will be determined once your income is presented.
