Q: What items do I need to bring to my appointment?
A: You must bring Identification to your appointment. If you do not have insurance, please bring proof of income so that we can verify prior to providing services including labs, x-rays and general visits. The following are acceptable items:
One Month's Income (Monthly paystub if paid monthly; 2 paystubs if paid bi-weekly; 4 weekly paystubs if paid every week.)
3-Months Bank Statements
SNAP Benefits Letter Showing Benefit Amount
Signature of Person Providing for Living Expenses on a form you can obtain from us. (They will not be responsible for your bill)
All items can be presented prior to your appointment or the day of the visit prior to being seen.
Q: How do I request a medication refill?
A: You can request a medication refill by calling our front desk at 259-3258. Please have the name of the medication you are requesting ready and know the pharmacy you wish to pick it up from. Please allow 24 hours for all medication refills to be processed.
Q: What if I need to reschedule an appointment?
A: If you need to reschedule an appointment, please call our front desk at 259-3258. Please give 24 hours notice to reschedule or cancel your appointment.
Q: What if I need medical assistance after hours?
A: Always call the office and verify that we are closed. If the office is closed, please call the Nurse Advice Line @ 1-800-394-2237.
Q: How often do I need to present my income?
A: Income should be updated every six months for the sliding scale
Q: What do I do if my child or myself needs vaccinations?
A: The Immunization Clinic is held on Tuesday from 9-11am and from 2-4pm. Walk-Ins, No appointments. For more information about vaccines available, please call (757)259-3258.
